How do I get a new phone on tab medium without agreeing to a new plan? I currently pay $60 and want to keep this plan...

I want to start a new tab medium and keep my $60 plan, but it keeps asking me to agree to a new plan. How can I start a new medium tab without agreeing to a new plan?

Best idea is to go into a store and get your new phone there.  Should be able to keep your existing plan that way.

Did you try login to self serve first and purchase your phone?

Did it ask you to agree to a new plan or agree to enter the new Tab system?
If you are on the old Tab system - accumulating positive tab, are/were receiving Koodo's contribution  or receiving  BYOD discount - then, you won't be able to keep that. (No more contribution by Koodo)

If you have $60 plan, you can use Tab M and keep your plan.

I asks me to agree to pay my tab balance of $12 (no problem) and then I need to check a box that says I need to "agree to the plan changes which mean your plan will no longer be offered. If you switch this plan today it means you won't be able to get it back." There is no way to proceed with the phone purchase without checking the box.

Requirement to use Tab M is that your plan have to be $40 or over. Your plan is $60 and you don't have to change it.
It could be system glitch since your plan is no longer available. I would contact Koodo and ask them to fix it on their end. You can send a PM thriugh FB using link on the bottom of this page.
If you want to keep your plan, dont agree to switch/change your plan.
