I'm paying for 2 phones (Nexus 5's) and opted for paperless billing. Today I received a paper bill in the mail and supposedly was billed $2.00 for it. Why should I be charged for something I opted out of?

Are you sure you opted for paperless billing? If yes, then you should give Koodo a call to see if everything's alirght on their said. Most likely, if it's an error you will get a refund!

Log into self serve and click on the "Change Bill Type"
It will show you that you have the paperless option or not.
Also if you switch to paperles recently, your bill might issued before your change.

Is this your first bill? Everyone gets their first bill from them in paper format, even if you signed up for ebill, but there shouldn't be a charge for it. It doesn't make sense as you said.
