Cannot change phone

I have a Nexus 5 with Koodo that no longer has a tab. I just purchased an iPhone7 with Tab medium. I am attempting to Change Phone, however, when I click that option, the website kicks me back to the Self-Serve page. Am I doing something wrong, or is there an issue with the site?
NOTE: I have to first select the Change Number option from self-serve, as there is no option to Change Phone directly from the Self-Serve page.

There is no need to change your phone. Just pop your SIM card in, and you're good to go. You may have to get a new Tri-SIM though if your previous one won't fit in the iPhone.

Also if you have do a sim swap you will have to send a private message to Koodo via Facebook Messenger or DM via Twitter or call 611 as the feature has been temporarily disabled for fraudulent activity recent.

Unfortunately, I can't swap the SIM, since the Nexus 5 uses a standard sim and the iPhone 7 uses a nano SIM. 

I didn't realize about his feature being disabled. I will check these options out.
