Can I combine my positive tab balance with my current phone trade in when buying a new phone

You should be able to.
Trade in have to be done at a store and upgrade your phone at a same visit 

What phone do you have currently? 
You don't get much value from trade in program. ( It's less hassle and maybe quicker)
If you sell it privately, you might get more value and use it for upfront payment.

I currently have a Samsung Galaxy S5 bought brand new from koodo around 3 years ago.

I was just thinking trade in because selling it privately probably won't net me much more money compared to the time and effort to arrange that.

You'll get about $75 from the trade in program where if you sell on kijiji for example you can get around $250.

That's not too bad actually.

I just did a quick kijiji search in my area and a few listing for S5's were there for around $180-$200 in 10/10 condition with the box and whatnot. Mine would probably be 7-8/10 with no extras (other than the charger).

So while I may be able to get a bit more on kijiji or something, I think that time and effort would be better used elsewhere :)) Thanks for the input though I do appreciate it!

The phone value is appraised based on the device age and condition.
So, You might get $50 or lower at Koodo store.(because of age...)

You can always keep your old phone as a back up/emergency :)

$250 is extremely optimistic for an S5. You're right bout your time being worth more than the marginal increase you may get on Kijiji.

My addition to this would be to take in your phone for appraisal, and then decide. Personally, I wouldn't bother trading it in if they give you less than $70 because you'll pay at least that if you need to get a quick backup in an emergency, and it will likely be inferior to the S5.
