Found some instructions. Not sure if you've already tried these steps.

1. From the home screen, tap the Phone icon
2. Tap the Menu icon
3. Tap Settings
4. Tap Calls
5. Tap Call Forwarding
6. Tap Always Forward
7. Enter the number you would like your calls forwarded to and then tap Turn On

Number to be forwarded should look like this: +15145551234

Not all phones have call forwarding in the settings menu.

When I switched from post-paid to prepaid (same handset, same number and location), the MMI codes for forwarding (*21*etc) stopped working and gave error messages instead. I suspect that call forwarding does not work on prepaid and that advertising that feature is in error.

I think you may be right Bob. TELUS mentions: Call Forwarding is not available for TELUS Prepaid accounts, so the same probably applies to Koodo.
