What is required to add a second line to an existing monthly plan (no tab). Sharing is not required, however I want the same agreement for monthly rate, and options on a 2nd (net new) phone line, with a new number. e.g. I want to expand my account/plan to 2 lines, or sign up for a 2nd plan with the same rate, terms, conditions, etc

You just sign up with Koodo like you normally would and tell them it is for a second line on an existing account.

The only issue is that you must choose a plan that is currently in the market. You will not be able to get a plan that is not currently available even if you are adding an extra line to your account

As long as your current plan is market there is no issue. Same thing when you buy anything else. I.e. I bouhgt a Sony TV on sale and now I want a second one for the same price even though sale is not on anymore. Not likely to happen.
