Your best bet is to hit up the large corporate stores in your area. They tend to have more experience with this task. Then you can get in and out without issue.

You have to be eligible first. http://help.koodomobile.com/getting-started-with-koodo/sim-cards-and-non-koodo-phones/how-do-i-know-if-i-am-eligible-to-get-my-koodo-phone-unlocked Then you can call in to Koodo and they can unlock. note that you will need to update via itunes to get the unlock. And please verify with a diff carrier's sim (not telus) to ensure the unlock is successful.

I thought you can get an unlock code in self-serve. Did this change?

I don't believe you can get the unlock code for iPhones in self serve.

iPhones dont have unlock codes. They can only be officially unlocked via iTunes. Androids have unlock coded which you can request an unlock on self serve. Either way. Always test your unlock to make sure it works. You dont want to be in a foreign country with another sim and realize the unlock didnt work

IPhones don't use a code like Android so you won't be able to get a code through self serve no.
