Charged $44 in call forward Even when Add on was active

I did had a $3 add on but still was charged for forwarding more than 40$ not sure why. My call was forwarded to a local Canadian number. I will attach a copy below of bill

Hi ABH - looking at your account, you have a $3 add-on, but it's for the international long distance saver, which gives you a discounted rate on all long distance calls. You in fact don't have call forwarding as an add-on. 

@Allan - the $0.50/min charge is the pay-per-use rate. Customers that have the $3 add-on don't get charged an additional rate.

@Ranjan - I did *21*18007014497# and hit SEND and did receive a message that enabled call forward. Did I miss a step which could have minimized the billing?

@Ranjan - I did *21*18007014497# and hit SEND and did receive a message that enabled call ...That's the correct procedure to forward calls, but since you don't have the add-on you were charged the pay-per-use rate of $0.50/min. Once you add-on the monthly feature for $3, you'll still forward using that method, but you won't be charged pay-per-use.

When you log into your self serve account, go to 'View Rate Plan'.
Once there, the first line under 'Change My Plan' should be 'Your monthly add-ons', and to the right of that it should say 'Manage' - click on manage.
From there, you can remove any add-ons you don't need (i.e. if you no longer need the $3 Intl. LD Saver feature), and also choose any new add-ons from the list below - select $3 Call Forwarding. 
Once selected, you'll be able to review your list of add-ons and confirm your selection.
