My tab is paid off but my monthly bill cost has not changed

Once the tab is closed I assumed the bill amount would be reduced by $15 each month after, but this has not happened (tab closed 3 months ago). Is this not correct?

Indeed that is not correct. So you're sure you have a zero tab balance and your total bill amount is still the same? If so, send a private message to Koodo via Facebook messenger or dm via Twitter to get it sorted out as it requires a Rep then
