You can talk to your store manager, they will be able to contact Koodo for you 🙂

You'll have to reapply for the discount.

If you call customer service they should be able to reinstate it. Retail support should also work. Telus retail support has always reinstated mine over the phone no issue. That's assuming you work in the cellular department.

As far as I remember, Loblaws management has no power regarding the discounts because Loblaws and Mobile Shop are separately owned. You'd have to talk to Mobile Shop staff about this issue. They would call channel support and see what's up. The discount lasts for 1 year and then you need to reapply for it so they can ensure you're still with the company. It might have just been coincidence that it fell off because plan changes shouldn't remove it, but I'm not sure if the program changed since I left Mobile Shop.

From what I remember when I signed up my father for the discount at the Mobile Shop, his supervisor had to fill out a form in order for him to qualify.

They might have changed the program since then because I never needed any Loblaws manager to sign the form. It specifically asks for the employee getting the discount to fill out the form and attach a copy of their latest pay stub, then I would give it to my manager at the kiosk and he/she would scan and email it.
