I have an additional $5 usage charge in my bill that I cannot explain as I thought my data would be shut off when it approached the limit.

If you look on page 3 of your bill, does it show the usage charge as data or calls?

Have you logged into your self serve and downloaded your detailed PDF ebill?

Plus for data to shut off you need to be on a shock free data plan AND have the open turned on in self-serve to stop data and require you to reply 'year' to continue data service.
