What is service request ID?

My phone was dead and I need service request ID for replacement form, but I can't find it.

Nasim, where did you get this form that you are you filling you out?
Here is Koodo's warranty and repair policy:

You can either take your phone in to the nearest Koodo store or if that is unavailable to you, you can request a return mailer.

My apologies for the delayed response, Nasim. Here's what I was able to find out. Customers receive a claim creation email with their Service Request ID number, if they provide their email address to the Asurion agent during the claim flow process. Meaning that a call to Asurion is required. If the customer has not provided an email address, the Service Request ID number will be given by the agent while on the call. The customer can also call Asurion back at any time to confirm the Service Request ID number. To submit a service request or to check the status of an existing service request, contact an Asurion representative during business hours at: [b]1-844-574-1839. Asurion customer service centre is open; Monday to Friday 8:00 a.m. to 12:00 a.m. Saturday 8:00 a.m. to 10:00 p.m. Sunday 12:00 p.m. to 10 p.m (all times in EST).
