Solved

We have two accounts, and the names and numbers are wrong. My name shows opposite my husband's number and vice versa. How can we fix this?

So you have 2 separate accounts, meaning 2 separate Self Serve logins?
Sorry, no.  One login, but 2 phones.  My husband's phone is less than 1 week old.  When I went to self help today, I see his name is next to my number (my phone is 10 months old) and my name is next to his.

When you oen your account/line, did you use your husband name and information?

Ok, 1 login = 1 account with 2 lines.

Was the name correct when you only had 1 phone?

At the end of the day, this error is superficial.  The name is just a label, and the name on the account for the 2 lines combined (on your downloadable pdf ebill) is most important as that is the main owner of the account who is responsible for all payments and owns both lines.
If you do what the names switched, I recommend contacting Koodo via private message on their twitter or facebook page and a rep can assist you with the changes

Agreed that it's just labels, unless you're trying to port out a number.

This only matters if you are on Fido or Rogers because they have WhoCalledTM.

And cellular caller ID name display. AFAIK, rogers/fido subscribers will not see name display on calls coming in from Télus/Koodo/Bell/Virgin subscribers.
