Question

I purchased a second phone for my spouce and I asked for a $30 plan also. BUT no plan was included Why ?

How do you know no plan was included?
On the invoice it says welcome letter no plan. Also the sim is a Koodo LTE trio sim and the phone says that it is available it wont connect.

If you're sure that there's no plan activated as well, it's possible that it was a mixup. You can go bakc to the location you got the phone and ask for it to be corrected.

Was this purchased online?  If so there could have been a delay activating the new plan.  I would wait a day or contact Koodo via private message on facebook or twitter
It was purchased on line so was the sim both came together in the same box. Also phone connects to Koodo 3G BUT when we try the 611 activation I'm told there is no network connection.

Sounds like the SIM has not yet been provisioned. You should receive an email with the information about the account, including the phone number assigned to the device. If you don't receive one in the next day or so, send the web store an email asking it be reviewed.
