3g/4g isn't working

I have tons of data left but whenever I disconnect from Wifi my phone goes offline.  There is no "sign into koodo" notification that usually pops up when I run out of data, and my chrome browser simply states "offline".  I do not know how to find out what Koodo's APN details are but from what I've seen the only difference is my proxy is set to 74.49.0.18.

Can anyone help?
